I have a Discord friend who works for a boutique publisher and her answer when I repeated the question about “18+ art book for a lower rated game” was “definitely not”. At least as far as the ESRB is concerned, anything sold with the game has to keep to the age rating or the rating will be moved up.
I originally thought this was only for downloadable content, as was most infamously seen with Oblivion, but it seems to also be true for physical goods too.
